Please arrive 15 minutes before your first appointment in order to complete the necessary paperwork and obtain your insurance information. You will need to bring identification, such as a driver's license or state ID card, as well as any applicable insurance card(s) and any co-pay and/or referral required by your insurance. It may help to have a list of your questions and concerns to discuss at your visit as well as the list of all medications that you are currently taking. - Insurance Plans:
We accept most insurance plans. Please call our office or check with your carrier to verify eligibility. If your insurance requires a co-pay or referral, you must present this at each visit. It is the patient's responsibility to monitor their referral requirements to ensure your are properly covered for your visit and services. If you have a question about the referral process, please contact us.
